| February 11, 2017

COSHOCTON – The Coshocton City Health Department and the Coshocton County General District in collaboration with the Ohio University’s Appalachian Rural Health Institute have developed a community health survey for Coshocton County.

The survey is being conducted to assess health concerns and opinions from residents of Coshocton County. The results of the survey will be used to develop a community health improvement plan to set goals for improving the health of all people in our county. Your input as a resident is extremely important.

The survey should take less than 10 minutes to complete and can be completed online using a computer or mobile device. The survey will remain open until Friday, Feb. 24. The following link provides direct access to the survey: http://tinyurl.com/hkxasue.
